What sets 4:8 Physio apart?
4:8 Physio provides one-on-one care built around thorough evaluation, ongoing assessment, patient education, and a plan that fits your goals. Without insurance dictating visit length or treatment choices, the team can focus on high-quality care, practical next steps, and a strong patient-provider relationship.
How are payments handled?
After your individual appointment, you will receive an invoice for payment. If you have questions about pricing, discovery sessions, evaluations, or ongoing visits, contact the clinic before your appointment.
Does 4:8 Physio accept insurance?
4:8 Physio is a cash-based clinic and does not have a direct relationship with insurance companies. Patients are responsible for the cost of care. Superbills may be available upon request for patients who want to submit documentation to their insurance provider for possible out-of-network reimbursement.
What is cash-based care?
Cash-based care means the patient pays the clinic directly instead of 4:8 Physio billing insurance. This model allows longer, individualized visits and keeps clinical decisions focused on the patient’s goals rather than insurance restrictions.
What questions should I ask my insurance?
- Do I have out-of-network coverage for physical therapy services?
- What is my yearly deductible, and how much remains to be met?
- How many sessions per year does my plan cover?
- What does my plan reimburse for common physical therapy codes such as evaluation, re-evaluation, manual therapy, therapeutic exercise, therapeutic activity, gait training, and neuromuscular re-education?
- Do I need prior authorization?
- How do I submit a superbill, and what is the submission deadline?
What is a superbill?
A superbill is an itemized document that lists services provided, care codes, and other treatment information. Patients may submit a superbill to their insurance company to request possible reimbursement.
Is reimbursement guaranteed?
No. Reimbursement depends on the insurance company and the patient’s specific benefits. 4:8 Physio cannot guarantee reimbursement and does not contact insurance companies to discuss benefits on the patient’s behalf.
How long does a superbill take?
Superbills are provided upon request. After a request is made, the clinic typically provides the superbill by email within 2-3 weeks.
How do I add childcare support to my visit?
Childcare support is available for infants through older kids and is included with visits. Contact the team before your appointment so they know your child’s age and any details that would help the visit feel smooth.
